I understand all reasonable safety precautions will be taken by Word of Life Church and its agents during events, trips and activities. I also understand the possibility of unforeseen hazards and know the inherent possibility of risk. I hereby agree to release and forever discharge, and agree to hold harmless, Word of Life Church, its leaders, employees, volunteer staff from any liability, and claims for damages, losses, sickness or injuries.

I understand that if medical attention is needed, for this child/ren during activities, every attempt will be made to consult the listed contact person on this form. If, however, the persons listed cannot be reached, I give my permission to the activity leaders to secure the serviced of a licensed physician or surgeon to provide medical services deemed necessary for the well-being of the child/ren.

During the KidzLife program year, we have opportunities to share the accomplishments and activities of our program participants. To do this, we may take photographs, voice recordings, video recordings and other visual documentation of our participants. WOLC and our teachers may share these images or recordings in church publications (both printed & digital).
Such publications may include, but are not limited to: publicly displayed photographs, advertising brochures, newsletters, announcement slide presentations, and any social media outlets. These publications become property of Word of Life Church.
Please indicate below whether you consent for these media images to be gathered and published, and your permission will remain in effect while your child/ren is/are enrolled in our program.
Also indicate if your consent has any exception. (photos limited or Facebook only etc.) Unless an end date is noted, WOLC may use the images and/or recordings indefinitely.
